Predictors of level V metastasis in well-differentiated Thyroid Cancer. Head Neck. 2008 Aug 14; Authors: Kupferman ME, Weinstock YE, Santillan AA, Mishra A, Roberts D, Clayman GL, Weber RS BACKGROUND.: Cervical lymphadenectomy is frequently performed in patients with lateral cervical lymph node metastases to improve regional control of disease. However, there is no consensus regarding the appropriate levels of the neck that need to be dissected. Treatment options that have been advocated include the modified radical neck dissection, limited neck dissections, and selective nodal excisions. In particular, the routine dissection of level V remains controversial due to the attendant morbidity to the spinal accessory nerve. To identify clinical and pathological predictors of cervical node metastases to level V in differentiated Thyroid carcinoma, we reviewed our experience at The University of Texas M. D. Anderson Cancer Center for the management of metastatic well-differentiated Thyroid Cancer. METHODS.: We retrospectively analyzed 70 patients who underwent thyroidectomy and neck dissection for well-differentiated Thyroid Cancer at M. D. Anderson Cancer Center. RESULTS.: In our series, 53% of neck specimens harbored metastatic Thyroid carcinoma at level V. We assessed the utility of GLV-1h68 in treating anaplastic Thyroid Cancer in vivo. Design: Athymic nude mice with xenograft flank tumors of human anaplastic Thyroid carcinomas (8505C and DRO90-1) were treated with a single intratumoral injection of GLV-1h68 at low dose (5x10(5) pfu), high dose (5x10(6) pfu), or PBS. Virus-mediated marker gene expression (luciferase, GFP and beta-galactosidase), viral biodistribution, and flank tumor volumes were measured. Results: Luciferase expression was detected 2 days after injection. Continuous viral replication within tumors was reflected by increasing luciferase activity to day 9. At day 10, tumor viral recovery was increased >50-fold as compared to the injected dose, and minimal virus was recovered from the lung, liver, brain, heart, spleen, and kidneys. Activation of the insulin/PI3K (phosphoinositide-3-kinase) signaling pathway in TSH-stimulated Thyroid cells reduces NIS expression at the transcriptional level. We, therefore, investigated the effects of PI3K pathway inhibition on iodide uptake and NIS expression in rat Thyroid cell lines and human papillary Thyroid Cancer cells. A PI3K inhibitor, LY294002, significantly enhanced iodide uptake in two rat Thyroid cell lines, FRTL-5 and PCCL3. The induction of NIS mRNA by LY294002 occurred 6 hours after treatment, and was abolished by a translation inhibitor, cycloheximide. Expression of the transcription factor, Pax-8, which stimulates NIS expression, was significantly increased in PCCL3 cells after LY294002 treatment. Removal of insulin abrogated the stimulatory effects of LY294002 on NIS mRNA and protein expression, but not on iodide uptake. MATERIALS/METHODS: 181 patients with differentiated Thyroid Cancer were included in the study. The nonstable disease group (recurrent and persistent disease) had 61 patients with 24-36 months follow-up period. The stable disease group (without recurrence or persistence) had 120 patients with 21-28 months follow-up period. We compared Tg and combined Tg-TgAb levels on the sixth month after the ablation between two groups. RESULTS: Optimal threshold values for Tg, TgAb measurements were calculated statistically as 4.45 ng/ml and 27.8 IU/ml. Optimal sensitivity and specificity for 4.45 ng/ml Tg levels were 52.5 and 91.7%. We also evaluated different Tg threshold values. We found sensitivity was 54.2% and specificity 86.7% for 2 ng/ml and sensitivity was 40.7% and specificity 96.7% for 10 ng/ml Tg levels.
