We present a case of psychotic unipolar depression in which the patient responded significantly to the adjunctive use of methylphenidate. A 45-year-old woman had melancholic depressive Symptoms and mood incongruent psychotic features during her second episode of unipolar depression. She attempted Suicide by hanging herself and was forcibly hospitalized. She was initially treated with venlafaxine (262.5 mg/d), olanzapine (20 mg/d), and benzodiazepines. However, she responded unsatisfactorily to the combination treatment. Because her family refused electroconvulsive treatment, we added methylphenidate to her medications for adjunctive use. The dose of methylphenidate was started at a dose of 5 mg/d. It was not until the patient received 30 mg/d of methylphenidate that her persistent psychosis and severe depression were substantially improved. Beyond the usual pain locations associated with abuse, 43.2% reported swollen/painful joints. More interference in daily life was attributed to joint pain than to back, head, stomach, pelvic or bowel pain. Women with high disability pain were more likely to have experienced child abuse, adult sexual assault, more severe spousal abuse, lifetime abuse-related injuries, Symptoms of depression and post-traumatic stress disorder, lifetime Suicide attempts, difficulty sleeping, and unemployment. High disability pain also was associated with visits to a family doctor and psychiatrist and use of medication in more than prescribed dosages. Less than 25% of women with high disability pain were taking opioids, or prescription nonsteroidal anti-inflammatory medications. A standardized abstraction sheet was used to collect data regarding the ED diagnosis at the time of enrollment, physician documentation of suicidal ideation, and referral to psychiatric services, as well as subsequent ED and clinic visits and Suicide attempts. RESULTS: Of the 165 patients who disclosed suicidal ideation on the computer survey, 118 charts (72%) were available. During the index ED visit, only 25% of patients had suicidal ideation or other mental health issues noted on the chart. METHOD: Women with PCOS (n=49) meeting the Rotterdam criteria for PCOS, and 49 age-matched controls identified from the population registry, were recruited. Trained clinicians used the MINI International Neuropsychiatric Interview to establish lifetime occurrence of Axis I DSM diagnoses. Serum-testosterone and sex hormone binding globulin were analyzed. RESULTS: Women with PCOS had higher lifetime incidence of depressive episodes, social phobia, and eating disorders than controls. Suicide attempts were seven times more common in the PCOS group than in the controls. Current as well as lifetime use of antidepressants and anxiolytic drugs were more common in the PCOS group. CONCLUSIONS: Previous studies have found that PCOS is associated with decreased quality of life and self-rated mental Symptoms. METHODS: Thirty-five Suicide attempters and 16 non-suicidal controls were admitted to a psychiatric ward between the years of 1986 and 1992. Corticotrophin-releasing hormone (CRH) in cerebrospinal fluid and urinary cortisol were obtained for the Suicide attempters. The patients were followed up approximately 12 years after the index admission. Cortisol was measured in saliva, and additional Suicide attempts and current psychiatric Symptoms were registered. RESULTS: At follow-up, evening salivary cortisol was lower in Suicide attempters compared to controls. Low cortisol levels at follow-up were associated with severe psychiatric Symptoms. In Hungary most of the treated patients are diagnosed with schizophrenia. Aim. To analyze the practice of referring patients for ECT in Hungary. Methods. Questionnaires containing socio-demographic data were mailed to all Hungarian psychiatric units where ECT was used (n=34), and all of the psychiatrists working there were invited to participate. Respondents were asked to rate how often they considered ECT for various Symptoms/syndromes on a five-point Likert Scale. Results. A total of 78 questionnaires were returned. Altogether, 89% of the respondents have referred patients to ECT, and 54.8% had done so in the last year. The spores that cause anthrax, Bacillus anthracis, occur widely in soil, and the skin form of the infection used to be common, especially among livestock and people who work with the animals. This cross-sectional study assessed the association of shift work with Suicide ideation among police officers.Shift work was based on daily payroll records over 5 years (41 women, 70 men). Standardized psychological measures were employed. ANOVA and Poisson regression were used to evaluate associations.Among policewomen with increased depressive Symptoms, prevalence of Suicide ideation increased by 116% for every 10-unit increase in percentage of hours worked on day shift (prevalence ratio (PR) = 2.16; 95% confidence interval (CI) = 1.22-3.71). Yesterday I was a panelist on a television program talking about the rising number of suicides and Suicide attempts in the American military. Being a veteran myself, and having acted as a veteran’s advocate in my undergraduate days vets issues have always been of special interest to me. So let me summarize some of the facts that you may be unfamiliar with. Currently, many veterans who served in Iraq and Afghanistan are suffering from invisible wounds. As in previous wars, service members can leave a war zone, but the war often follows them in their minds. Numbers are always iffy but according to a RAND study released in April, nearly one in five Iraq and Afghanistan veterans report Symptoms of post traumatic stress disorder (PTSD) or major depression. It estimated that 830,000 veterans - 300,000 of whom served in Iraq or Afghanistan - suffer from depression or PTSD Symptoms. Further, the study found that the stigma associated with mental healthcare, and concerns that admitting a problem will harm one’s career, continues to prevent many service members and veterans from seeking care. The study found that only half are seeking treatment, and only half of these “receive treatment that researchers consider ‘minimally adequate’ for their illnesses.” The study estimates that PTSD and depression disorders among veterans cost the nation as much as $6.2 billion in the two years following deployment—an amount that includes both direct medical car ...

Huntington's Disease is a serious, progressive disease that involves degeneration of part of the brain.  In particular, there is loss of neurons and development of gliosis in the striatum.  The disease is named after George Huntington , who  described it in 1872. Huntington's Disease can be diagnosed by genetic testing .   There has been a lot of progress in understanding exactly what goes on in the brain , as the disease progresses.  Still, nothing close to a cure has been found.   It is possible to treat some of the Symptoms.  Up until now, such treatments have been only minimally effective.  Death usually occurs 10 to 20 years after Symptoms first appear.  Death usually occurs due to pneumonia.  The Suicide rate are 7 to 10 times greater than that in the general population. In some non-US countries, tetrabenazine ( Xenazine ) has been available for a while (35 years).  However, the FDA in the USA requires pre-approval testing that is more stringent than in most countries.  It has been difficult to do testing that is acceptable to the FDA, because good testing requires large numbers of patients.  The disease is sufficiently rare (4 to 10 cases per 100,000 persons), that it is a challenge to get enough patients.  Also, the disease progresses slowly, so there isn't a quick way to know if the drug is working.
