RESEARCH AND DESIGN METHODS: This was a randomized, double-blind, parallel group, multi-center trial in adolescent and adult subjects >/=12 years of age symptomatic on a low dose of an inhaled corticosteroid (ICS). The study consisted of a 2-week screening period on low dose ICS; a 4-week open-label FP 250 mcg twice daily (BID) run-in; a 52-week double-blind period (FP/Salmeterol [FSC] 100/50 mcg [n=239] or FP 100 mcg [n=236] BID), and a 4-week FP 250 mcg BID run-out period. Annualized exacerbation rate was the primary outcome for comparing the two treatments. Other measures of asthma control included peak expiratory flow, asthma symptoms, and albuterol use. Recently, we have demonstrated that combination of Salmeterol and fluticasone propionate (FP) additionally suppress the production of IL-8 by human monocyte. In this study, the molecular mechanism behind the effectiveness of this combination therapy is investigated in human neutrophils. Human neutrophils were preincubated with Salmeterol or FP or the combination. The amount of interleukin-8 (IL-8), elastase and matrix metalloproteinases (MMP)-2 and -9 releases, and reactive oxygen species (ROS) generation and expression of MAP kinase phosphatase (MKP-1) and glucocorticoid receptor (GR) were determined. Cigarette smoke medium (CSM) induces an increased expression of CXC receptors and the production of ROS that may explain the strong production of IL-8 by neutrophils. I found an interesting article from MSN Health & Fitness regarding Eight Drugs Doctors Would Never Take . Most interesting to me was that two of the drugs listed were drugs I've taken in the past and never had any idea I shouldn't have taken them. More importantly -- they are still on the market and you may also be taking them. The eight drugs and why you shouldn't be taking them are: 1. Advair - asthma medicine. Advair contains LABA, the long acting beta-agonist Salmeterol. Apparently, regular use of LABAs can increase the severity of your next asthma attack. 2. Avandia - diabetes medicine. Also known as rosiglitazone - taken for at least a year can increase your chance of heart failure or a heart attack by 109% and 42% respectively. 3. Celebrex - nicknamed "super aspirin". Also, known for it's side effects of increased risk of stomach bleeding, kidney trouble and liver damage. Celebrex is still available, although two other COX-2 inhibitors, Bextra and Vioxx were pulled off the market. 4. Ketek - used for respiratory infections can cause heart rhythm problems and can lead to liver disease. 5 and 6. Prilosec and Nexium - proton-pump inhibitors - are very effective at stopping acid production in your stomach. A lack of acid can increase your risk of pneumonia.
