This non-inferiority design trial (registered as ISRCTN 39679215) aimed to determine whether gemcitabine and carboplatin (GC) has a similar survival but is less toxic with better quality of life (QoL). METHODS: Previously untreated SCLC patients with extensive disease or limited-stage with poor prognostic factors were randomly assigned to six, 3-weekly cycles of GC, or PE. RESULTS: 241 patients (121 GC and 120 PE) were recruited of which 216 (90%) had died. There was no difference in overall survival (HR 1.01, 95% CI 0.77 to 1.32). Median survival was 8.0 months and 8.1 months with GC and PE respectively.. Median progression-free survival was 5.9 months with GC and 6.3 months with PE. Grade 3 or 4 myelosuppressions were more frequent with GC (anaemia, 14% GC v 2% PE; leucopenia, 32% GC v 13% PE; thrombocytopenia, 22% GC v 4% PE) but these were not associated with increased hospital admissions, infections, or fatalities. MATERIALS AND METHODS: We examined the efficacy and toxicity of thalidomide, an anti-angiogenic agent, in combination with carboplatin and etoposide and as maintenance therapy in patients with untreated SCLC. Twenty-five chemotherapy-naive patients with extensive disease (ED) or limited disease (LD) SCLC were enrolled in a single-arm phase II study. Carboplatin and etoposide were given every 3 weeks for 6 cycles with concurrent thalidomide 100mg orally daily. The treatment with thalidomide was continued as maintenance for up to 2 years. RESULTS: Median progression free and overall survival were 8.3 months and 10.1 months, respectively. One-year survival was 40% and the 1-year progression-free survival was 36%. The overall response rate was 68% (95% CI 46-85%) with four complete remissions (20%) and 13 partial remissions (48%). We observed no increase in chemotherapy related toxicity. Thoracic radiation, afterloading and radionuclide therapy supplemented the therapeutic management. Remarkably, he had two very long remission periods following topotecan. Conclusion: This case indicates that frequently repeated chemotherapy beyond second-line treatment in a subset of patients with limited-disease SCLC may result in long-term survival, even without ever achieving a complete remission. This study cross-validates the PSALC using oral topotecan (OT) trial data. METHODS: Data were analyzed from a randomized, open-label, multicenter trial including 71 patients with relapsed SCLC receiving OT with best supportive care and 70 patients receiving best supportive care alone. PSALC and EQ-5D were administered at baseline and at 3-week intervals. Internal consistency, reliability, construct validity, and responsiveness were evaluated. RESULTS: Only one factor was indicated in factor analysis, hence PSALC total score (PSALC-TS) was used for psychometric analysis. Internal consistency was supported by Cronbach's alpha of 0.78. The Rev. Simmie Lee Harvey, a civil rights stalwart who worked alongside Martin Luther King Jr. and helped plan the March on Washington in 1963, has died. He was 90. Harvey died Sept. 10 of complications from a stroke, according to Rhodes Funeral Home. He had been hospitalized for about two weeks before his death. Harvey was a co-founder of the Southern Christian Leadership Conference, which was created in 1957 by King and other leaders from across the South with the purpose of advancing racial equality. "Rev. Harvey was one of the original leaders of SCLC," Dr. Joseph Lowery, a former president of the group for 20 years said in a statement. "He was a genuine and authentic person. He wore the badge of SCLC on his coat and in his heart." Harvey helped plan the March on Washington, which culminated with King's "I Have a Dream" speech. ...
