AANA J. 2008 Jun;76(3):209-12 Authors: Perkins PE High-frequency jet ventilation (HFJV) has been used in emergency airway scenarios and various surgical procedures. Although substantial literature is available regarding HFJV in these situations, there is only 1 publication to date concerning its use for cardiac Radiofrequency Ablation procedures. The following case study describes a 49-year-old man undergoing Radiofrequency Ablation in which HFJV was used. This method has been used for these procedures for months in our institution with great success. Its effectiveness is attributed to the lack of significant heart movement as compared with conventional intermittent positive-pressure ventilation, which, in turn, has improved surgical conditions and resulted in decreased procedure times. In this case, a newly introduced in-line circuit filter was used. Impedance to passive exhalation was created after the filter became saturated from the high humidification. This event, its management, and the following discussion on the mechanics of HFJV and its use in Radiofrequency Ablation procedures make this case an educational value to all anesthesia providers. The sequential combination of these two modalities has a sound theoretical basis and raises the probability of complete local response in tumors that are beyond the size at which the likelihood of failure with RFA alone begins to rise. Cheng et al. have found that when limited to patients with well-compensated liver disease, combined TACE and RFA does not seem to dramatically increase the risk of complications beyond the risk levels associated with the individual procedures. The survival benefit demonstrated in this recent article is, however, an artifact of a rigid study design that did not allow rational selection of treatment on the basis of individual patient and tumor characteristics. This study bolsters the evidence that the combination of TACE with RFA enhances local response, strengthening the status of nonsurgical treatment compared with resection for the treatment of early HCC. WINSTON-SALEM, N.C. – Studies of patients with kidney cancer have shown that Radiofrequency Ablation, a minimally invasive, kidney-sparing procedure, can be a successful treatment option for patients whose cancer has not spread beyond the kidney. The treatment uses computed tomography for guidance during percutaneous placement of a needle-shaped electrode into the tumor. Radiofrequency current is then passed through the electrode to heat the tumor tissue and ablate – or eliminate – it. In the majority of patients complete tumor Ablation can be achieved after a single treatment; others achieved complete tumor Ablation after two or more treatments, with no long-term or serious complications. "This is significant and encouraging because the incidence of kidney cancer in the United States has increased by 126 percent over the past 50 years," said Ronald J. In treatment using the VNUS Closure system (VNUS Medical Technologies, Inc., San Jose, CA, USA), varicose veins are closed endovenously rather than removed surgically. A fine catheter with a RF delivery element (electrodes) at its tip is inserted distally and threaded up the vein. The electrodes deliver the RF energy into the vein, causing resistive heating of the vein wall. The heat applied within the vein causes the collagen in the vein wall to be denatured and contract, effectively closing the vein. The endothelial cells of the vein are destroyed, preventing regrowth. The lumen of the vein is sealed with fibrotic protein, and the vein may eventually disappear altogether. In practice, the VNUS Closure System is used primarily on the GSV. Curative and palliative resection, which is possible in less than 20-25% of the patients, relieve endocrine and local symptoms in 90% of the patients for more than two years, and the five year survival is prolonged to 40-85%, although metastases recur or progress in almost all patients. Tumour Ablation by Radiofrequency therapy has a palliative effect on endocrine symptoms in 70-90% of the patients for up to two years, but should not be a substitute for surgical treatment. When metastases are not eligible for surgical treatment or Ablation, embolization or chemoembolization are alternative options with a reduction in tumour burden in about 50% and a five year survival of around 60% ofthe patients. Recent multicenter study comparing laparoscopic partial nephrectomy and open partial nephrectomy demonstrated that equivalent cancer-specific survival was similar between the two modalities. Encouraging long-term data are available for laparoscopic partial nephrectomy and cryoablation. However, some concerns remain about incomplete tumor cell kill after Radiofrequency Ablation. Radiosurgery is a promising new technology, but is still experimental.
