The present study reviews our experiences with endoscopic thyroidectomy using bilateral breast areola and ipsilateral axillary (BBIA) approach to evaluate its safety and feasibility. METHODS: From June 2003 through November 2007, the study group was comprised of 68 consecutive patients with benign thyroid nodules (66 women; mean age, 33.28 +/- 10.3 (range, 15-72) years). A detailed description of the surgical technique is provided. RESULTS: The mean maximum diameter of the tumor was 3.14 +/- 1.61 (range, 1-10.7) cm. Sixty-five cases were successfully resected with an endoscopic procedure, but three cases were converted to a conventional open procedure due to bleeding. Endoscopic procedures were 61 lobectomies, 2 isthmectomies, and 2 near total thyroidectomies. The mean operating time was 89.93 +/- 24.71 (range, 50-170) minutes, and the last 32 cases tended to take less time than the first 33 cases (P = 0.033). This novel material was then coated on a quartz crystal microbalance (QCM) to investigate its Gas sensitive properties to the Nerve agent simulant dimethyl methylphosphonate (DMMP) vapor, as well as known interfering vapors. When tested with competing vapors, the sensor was more than 10 times sensitive to DMMP than to other interfering vapors. Thus, high selectivity of poly{methyl[3-(2-hydroxy-3,4-difluoro)phenyl]propyl siloxane} to DMMP was demonstrated. The poly{methyl[3-(2-hydroxy-3,4-difluoro)phenyl]propyl siloxane}-QCM sensor responded linearly to DMMP vapor with a slope of 14 Hz/ppm in the 1-50 ppm range with a detection limit of 0.21 ppm (S/N=3). Barack Obama never suggested that proper inflation of tires would make the U.S. energy independent, just that it would be smart for consumers feeling pinched by high Gas prices and that it would lead to consuming less gasoline. Based on some studies that look at how under-inflated tires can be in the U.S., many consumers could save around $500 a year at current Gas prices if they check their pressure at every fill-up. About a month ago we promised to get our hands on some scooters for evaluation. We’ve just added a couple of Vespas to our test fleet, a 50cc LX50 and a 150cc LX150. The LX50 model cost us $3,299, the LX150 $4,399. After some buzzing around (“Vespa” means wasp in Italian) in the immediate vicinity, it became clear that most of us prefer the 150. Though the 50cc model doesn’t require a motorcycle license, it’s limited to only 30 mph and feels sluggish. In order to ride legally and confidently on highways, you’d need a 250cc scooter. The other day I ferried the LX150 home on a trailer so I could ride it in more urban and suburban settings than the rural roads around our test track. I immediately felt very trendy. Spurred on by record high Gas prices, the Vespa has become a fashion statement in my neck of the woods in Fairfield County, Connecticut. Mind you, I’m not a two-wheeler novice: I used to own a Triumph motorcycle. It took a bit of a mindset adjustment to ride the Vespa, but I’m happy to announce that my ego remained intact.
