We also focus on the question of how these concepts have dealt with the particular problems posed by prognosis and outcome. Althought different terms have been used, a robust description of delirium has existed since antiquity--at some times as a symptom and at others as a syndrome. It is clear that, throughout the millennia, delirium has been--and still is--a highly lethal syndrome; a poor Mental outcome for survivors was often noted. Not until the twentieth century was it thought that delirium was marked by a full recovery among survivors, and this was probably due to the desire for a clear distinction from dementia. Data from the nationally-representative National Comorbidity Survey Replication was examined. Multiple logistic regression was used to determine whether mood, anxiety, impulse control, and substance use disorders were associated with a higher rate of potentially violent behavior as assessed by threatening others with a gun or other weapon. After adjusting for sociodemographic factors, an association was found between mood, anxiety, impulse control, and substance use disorders and the rate of threatening others. A significant association was found between threats made against others with a gun and both substance use disorders (adjusted odds ratio [AOR] 2.27; 95% confidence interval [CI] 1.62-3.20) and impulse control disorders (AOR 2.67; 95% CI 1.95-3.66). Threats made against others with any other type of weapon were significantly associated with any anxiety (AOR 1.76; 95% CI 1.34-2.31), substance (AOR 2.63; 95% CI 1.87-3.71), or impulse control Disorder (AOR 2.49; 95% CI 1.96-3.18). CBT was compared with treatment as usual (TAU) in a randomized controlled trial with 108 clients with PTSD and either major mood Disorder (85%) or schizophrenia or schizoaffective Disorder (15%), of whom 25% also had borderline personality Disorder. Eighty-one percent of clients assigned to CBT participated in the program. Intent-to-treat analyses showed that CBT clients improved significantly more than did clients in TAU at blinded posttreatment and 3- and 6-month follow-up assessments in PTSD symptoms, other symptoms, perceived health, negative trauma-related beliefs, knowledge about PTSD, and case manager working alliance. The effects of CBT on PTSD were strongest in clients with severe PTSD. Homework completion in CBT predicted greater reductions in symptoms. Subjects provided retrospective reports of lifetime Mental illness (including SAD) and concurrent reports of current Mental illness at age 16 and were then followed prospectively until age 30. Diagnostic assessments were conducted twice during adolescence and again at ages 24 and 30. Based on diagnosis during childhood/adolescence, the subjects were partitioned into four orthogonal groups: SAD (n = 42), other anxiety disorders (n = 88), a heterogeneous psychiatric disorders control group (n = 389), and a not mentally ill control group (n = 297). Adjusting for demographic variables that were significantly associated with group status and for comorbid disorders prior to age 19, the results were analyzed with hierarchical multiple logistic regression. Mouse study offers hope for correcting autism's disruption of the brain UCLA researchers have discovered that an FDA-approved drug reverses the brain dysfunction inflicted by a genetic disease known as tuberous sclerosis complex, or TSC. And because half of TSC patients also suffer from autism, the findings offer new hope for addressing learning disorders associated with autism. The findings appear in the June 22 online edition of the journal Nature Medicine. TSC is a devastating genetic Disorder that disrupts brain function, often causing severe Mental retardation. Even in mild cases, learning disabilities and short-term memory problems are common.
