Am J Surg. 2008 Oct;196(4):575-7 Authors: Sookhan N, Boughey JC, Walsh MF, Degnim AC BACKGROUND: Nipple-sparing mastectomy (NSM) combines skin-sparing mastectomy with preservation of the nipple-areolar dermis and intraoperative pathologic assessment of the nipple core. We evaluated our initial experience with NSM in terms of clinical outcomes. METHODS: An Institutional Review Board-approved retrospective review of patients undergoing NSM between November 2005 and June 2007 was performed. RESULTS: Eighteen NSM and two areola-sparing Mastectomies were performed. Indications for surgery were invasive cancer (n = 4), ductal carcinoma in situ (DCIS) (n = 5), pseudoangiomatous stromal hyperplasia (n = 3), and risk reduction (n = 8). The average distance of tumor from the nipple on imaging was 4.8 cm (range 4 to 5.7). Nipple cores were all benign, and 2 patients developed self-limited superficial desquamation of the nipple. At a mean follow up of 10.8 months, all nipple-areolar complexes were intact, and there were no local or systemic recurrences. The aims of this study were to determine the surgical outcome of laparoscopic oophorectomy when carried out by breast surgeons and whether laparoscopic oophorectomy can be safely carried out during the same anaesthesia as breast surgery. Methods: Patients who had laparoscopic oophorectomy carried out by two breast surgeons were reviewed with regard to the indication, surgical outcome and concurrent procedures. Salpingectomy was also carried out when the indication was prevention. Results: Seventy patients with breast disease had laparoscopic oophorectomy between January 2000 and June 2007. Forty-three patients had laparoscopic oophorectomy for adjuvant endocrine treatment of early breast cancer, 13 for prophylaxis, 7 for endocrine and prophylactic reasons and 7 for treatment of metastatic breast cancer. Sixteen patients had laparoscopic oophorectomy and breast surgery at the same time, without complication. J Surg Oncol. 2008 Aug 14;98(3):197-199 Authors: Chen SY, Lin CH, Chang HM, Hsu HM, Yu JC BACKGROUND: The purpose of this study was to develop a modified method to implant a totally implantable access port (TIAP) using the femoral vein approach. METHODS: We designed a modified method using the femoral vein approach to implant a TIAP in patients with synchronous bilateral breast cancer requiring bilateral mastectomy and postoperative chemotherapy. TIAP implantation was performed with parenteral sedation and local anesthesia in the operating room. All patients were followed for at least 12 months and the complications of TIAP were recorded. RESULTS: In this retrospective study, 86 patients received the TIAP using the modified femoral vein approach. All patients had a history of bilateral breast cancer and underwent bilateral mastectomy. The early complication rate within the first 30 postoperative days was 2.3% and involved groin hematoma caused by missed puncture to the femoral artery during the operation. The late postoperative complication rates were 2.3% caused by local port infection, 1.2% by groin wound infection, and 3.5% by catheter occlusion. She is carrying the potentially deadly BRCA gene mutation , which gives her chilling odds of breast or ovarian cancer during her lifetime. (It's a diagnosis shared by actress Christina Applegate -- who did have early-stage cancer in one breast -- and "Gossip Girl" writer Jessica Queller, who recently wrote about the experience in her memoir, "Pretty Is What Changes." Congressman Brian Higgins (NY-27) voted with his colleagues in the House of Representatives to approve H.R. 758, the Breast Cancer Patient Protection Act. The legislation, cosponsored by Congressman Higgins, aims to end the practice commonly known as “drive through Mastectomies.” “Too often patients who have undergone major invasive procedures are forced by insurance companies to leave the hospital before they and their doctors feel they are ready,” said Congressman Higgins “This legislation puts cancer treatment and prevention decisions back into the hands of those best able to make those judgments.” Under the bill patients are guaranteed a minimum hospital stay of 48 hours for mastectomy or lumpectomy and a 24 hour stay for those undergoing lymph node removal. The legislation by no means mandates a 48 hour stay or sets 48 hours as the maximum stay. In addition, the Act requires health plans to provide full coverage for patients seeking second opinions and distribute information telling patients about their rights related to hospital stays for these treatment ...
