Arthritis Rheum. 2008 Sep 29;59(10):1475-1479 Authors: Scofield L, Reinlib L, Alarcón GS, Cooper GS OBJECTIVE: To summarize research pertaining to work disability in Lupus patients, discuss challenges patients face applying for federal disability assistance in the US, and make recommendations for clinical and health policy research. METHODS: We searched Medline for articles on work or disability in Lupus patients and gathered information from the Social Security Administration and the National Organization of Social Security Claimants' Representatives. RESULTS: We found 12 publications with employment-related data; 6 included analysis of predictors of work status. The prevalence of inability to work or cessation of work was 15-51% in these studies (3-15 years after diagnosis); 20-32% of patients received disability benefits. Lower education level, higher disease activity, higher disease damage, older age, and higher physical job strain were independent predictors of work disability or work cessation in at least 2 studies. Lupus patients may be less successful than patients with other diseases when applying for federal disability assistance, possibly because medical records may not accurately reflect functional limitations. The level of Tregs in peripheral blood was detected by flow cytometry. The expression levels of GITR and Foxp3 mRNA in peripheral blood mononuclear cells (PBMCs) were assayed by reverse transcriptase-polymerase chain reaction (RT-PCR). The level of IL-6 in the plasma was measured by ELISA. Comparisons were made among 3 groups: the active SLE group, the inactive SLE group, and normal control group. The level of Tregs in the active SLE group and the inactive SLE group was significantly lower than in the normal control group (P<0.01). The level of Tregs in the active group was lower than in the inactive group with the difference being not significant (P>0.05). The level of Tregs in SLE patients was significantly negatively correlated with the disease active index in SLE (SLEDAI) (r=-0.81, P<0.01). METHODS: Each IRF5 polymorphism was genotyped in 1565 subjects using the TaqMan method and examined to determine whether it could explain the association with SLE. RESULTS: Three single-nucleotide polymorphisms (IRF5-15-1, rs2070197, and rs10488631), which showed strong and/or independent association in Caucasian populations, were not polymorphic in our Korean population. Association analysis revealed different genetic effects in Koreans compared with Caucasian populations. In addition, conditional analysis suggested independent genetic effects of 3 variant groups in the Korean population. The patient was a 30-year-old woman who was diagnosed with SLE at the age of 25 and had been medicated with corticosteroid. At the age of 30, she was re-admitted into our hospital because of the relapse of Lupus nephritis. Renal biopsy was performed twice: at the onset of SLE and the second admission. Morphologically, the results of the first and second renal biopsies were compatible with the classification of Lupus nephritis class II. Immunofluorescent study revealed the mesangial deposition of IgG, IgA, C1q, C3 and membrane attack complex (MAC) in two renal biopsies. Especially, in the second renal biopsy, numerous vesicular structures composed of a unit membrane going into the epithelial site of the GBM were observed by electron microscopy (EM), but not in the first renal biopsy. Moreover, observations of serial sections of EM suggested that these vesicular structures were invaginated into the GBM and derived from a part of the podocytes. In this report, a CT scan was effective in demonstrating both the presence and the extent of inflammation of Lupus profundus in two patients with systemic Lupus erythematosus (SLE). Case 1 was a 30-year-old woman developing erythema with subcutaneous induration on the upper arms during the quiescent phase of SLE. A skin biopsy confirmed a diagnosis of Lupus profundus. A CT scan of the right upper arm demonstrated a high density area (HDA) of the subcutis under the erythema: a finding consistent with Lupus profundus. Case 2 was a 28-year-old woman recently diagnosed with SLE. She also developed a skin ulcer on the right hip. A CT scan of the hip revealed an HDA and lipoatrophy of the subcutis around the ulcer: these findings were compatible with Lupus profundus. Treatment with high-dose prednisolone improved the illness in the both cases. A CT scan is a useful and convenient imaging modality for confirming the diagnosis of Lupus profundus.
