Proteome Analysis of Chronically Inflamed Lungs in a Mouse Chronic Asthma Model. Int Arch Allergy Immunol. 2008 Jul 2;147(3):179-189 Authors: Wong WS, Zhao J Background: Asthma is a chronic airway inflammatory disease characterized by airway wall remodeling. The mechanisms underlying airway remodeling in asthma are not fully understood. There is an urgent need to investigate global protein profiling of chronically inflamed Lungs to identify novel pathogenic molecules and biomarkers for chronic asthma. In this study, we described the first differentially expressed proteome of lung tissue and bronchoalveolar lavage fluid from a mouse chronic asthma model. Methods: BALB/c mice sensitized to ovalbumin were challenged with ovalbumin aerosol 3 times per week for 8 weeks. The lung tissue and lavage fluid proteins were resolved by 2-dimensional gel electrophoresis, and differentially expressed proteins were identified by matrix-assisted laser desorption/ionization time-of-flight tandem mass spectrometry. Results: Airway goblet cell hyperplasia, smooth muscle hyperplasia, subepithelial fibrosis, airway hyperresponsiveness, pulmonary inflammatory cell infiltration and elevated serum ovalbumin-specific IgE level were observed in our chronic asthma model. Studies dealing with constitutive VE-cadherin expression patterns in different pulmonary vessel types (arteries, arterioles, capillaries, venules, veins) or with the influence of physiological factors such as age or sex on VE-cadherin expression have not been published yet. Knowledge of constitutive resp. varying expression patterns not only fundamentally contribute to understanding the role of VE-cadherin in the pathogenesis of pulmonary diseases but also help to develop therapies based on immunotargeting. Hence, endothelial VE-cadherin expression was studied in regular lung tissue. Fifty-eight specimens of regular lung tissue (30 females, 28 males between 1 month and 75 years old) were immunohistochemically stained with an antibody against VE-cadherin. There was strong endothelial expression of VE-cadherin in arteries, arterioles, and capillaries but almost no expression in veins and venules. METHODS: This report describes the results of strategies that have evolved to evaluate and optimize cadaveric donor lung function and intra- and post-operative ICU management. Ongoing interactions between the intensive care unit (ICU) staff, donor coordinators and the LTx team all contribute to this process. Data are compared with the annual reports of the 2005 USA Organ and Transplant Procurement Network and UK Transplant. RESULTS: In 2001, 41% of all local (State of Victoria) multiple-organ donors referred for LTx were transplanted at the Alfred Hospital, with 36% considered functionally unusable. In 2006, 66% of Victorian donors contributed Lungs for LTx (18% functionally unusable, 16% logistically unusable). Of the interstate (rest of Australia) lung donor offers, 50% (no local LTx unit present) and 32% (local LTx unit with first offer) were utilized, with 33% and 48% functionally unusable, respectively. Of the 47 resultant Alfred Hospital LTxs in 2006, survival rates were 100% at 180 days and 96% at 365 days, with no mortality directly attributable to donor quality. We investigated how the expression of these molecules could change after cardiac arrest in a mouse lung, comparing other molecules presumably relating with ischemia. METHODS: At given intervals after cardiac arrest, the Lungs were harvested. Quantitative analysis of mRNA expression of claudin-5, VE-cadherin, IL-1beta, IL-10, HIF-alpha, Egr-1, VEGF, Ang-1 and Ang-2 genes in lung tissues with several periods of warm ischemia was performed. RESULTS: Regarding endothelial cell-specific molecules, there were significant differences in both claudin-5 and VE-cadherin mRNA expression between 0 h and 4 h after cardiac arrest. IL-1beta mRNA expression 1 h, 2 h and 4 h after cardiac arrest increased significantly, compared with that at 0 h. There were no significant differences with the other genes. MATERIAL AND METHODS: TBB of the Lungs was performed in bronchofibroscopy under local anesthesia and TV x-ray control with intake of 4-5 biopsy specimens. RESULTS: Biopsy specimens consisted primarily of alveolar tissue in 62.5% patients, of bronchial wall - in 34.8%, were insufficiently informative - in 2.4% patients. Histological examination of the biopsies detected sarcoid granulemas which served the basis for RS diagnosis in 62.5% patients (stage 1 - 51.1%, stage 2 - 70.4%, stage 3 - in 61.6%). Sarcoid granulemas were detected more frequently in alveolar tissue than in bronchial walls (75.3 and 38.5%, respectively, p < 0.05). In RS stage 1 granulemas in the bronchial walls were found less often than in stages 2 and 3 (56.3, 42.9%, respectively, p < 0.01). CONCLUSION: TBB provides morphological RS diagnosis in 2/3 patients and is a safe method. Obtaining biopsies containing alveolar tissue raises efficacy of TBB, especially in the disease stage 1.
