This innovative route of Insulin delivery offers the potential of administering pre-meal Insulin in a non-invasive way to patients currently receiving multiple daily injections. This article describes the pharmacodynamic and pharmacokinetic profiles and the efficacy and safety data of Inhaled Insulin preparations. Particular emphasis is placed on Exubera, which currently has the largest pool of efficacy and safety data. In patients with type 1 diabetes 24-week trials have demonstrated that Inhaled Insulin was equally efficacious to short acting Insulin (2-4 daily injections). Results of trials conducted in type 2 diabetes showed Inhaled Insulin efficacy too, and a potential role for Inhaled Insulin in patients failing oral medications. Safety data have shown that the most common reported adverse event is mild cough, which appears to be decreasing in frequency during the course of therapy. Higher antibody titers have been observed in patients treated with Inhaled Insulin compared to subjects treated with sub-cutaneous Insulin. Results- A1C reduction from a mean baseline of 7.9% was greater with TI than with TP (-0.72% vs. -0.30%; P=0.003). Postprandial glucose excursions were reduced by 56% with TI compared with baseline, and maximal postprandial glucose levels were reduced by 43% compared with TP. Mannkind took another big step toward filing an application for its Inhaled Insulin after releasing data from a late-stage trial demonstrating the therapy is not inferior to injected drugs that control blood sugar levels in Type 1 diabetes patients. Just as importantly, researchers noted that there was no difference in lung function measures for patients taking Mannkind's therapy, a safety factor that has dogged similar therapies. Mannkind has stuck with its Inhaled Insulin program even after Pfizer, Eli Lilly and Novo Nordisk had all walked away from their own efforts in the field. In a separate announcement Pfizer said that some Exubera patients who still need an Inhaled Insulin will be switched to Mannkind's therapy. Investors have battered the company's stock price, but the CEO says Mannkind plans to go the distance. "Technosphere Insulin promises to be an important additional option for the treatment of patients with Type 1 diabetes," MannKind Chief Scientific Officer Peter Richardson said in a statement. Call your Realtor: Pfizer has put its ex-Exubera plant up for sale. The Terre Haute, Indiana, facility had been in the Pfizer family since 1948, when the drugmaker bought it from the U.S. government. Since then, it's undergone a variety of expansions and upgrades, including more than $300 million in investment since 1999. At its peak, the 580,000-square-foot facility on almost 1,000 acres employed some 800 people. Most recently, the plant was used to make Exubera, Pfizer's Inhaled Insulin product. But just one year into the product's launch, Pfizer pulled the plug on the formulation because of lackluster sales. Exubera just hadn't caught on with patients, who disliked the oversized inhaler device. And payers were slow to adopt the Inhaled formula, too.
