Curr Drug Saf. 2008 Jan;3(1):35-45 Authors: Garcia-Marcos L, Ros-Lucas JA, Sanchez-Solis M Inhaled corticosteroids (ICS) are the cornerstone of asthma management both in adults and in children. There are some adverse effects related to the use of these drugs in all ages. Those adverse effects can be local or systemic. From the paediatric point of view, the main worry relates to the effect on growth and on the integrity of the HPA-axis. At the recommended doses, their effect on the latter is not clinically relevant and the slight modification of cortisol levels which occurs while taking them reflects their presence in blood. Although there is a slowing down on growth velocity, this is reduced to the first months of treatment which are followed by a catch up: there is quite consistent data supporting their lack of significant effect on the final height. Other adverse effects which may appear in relation to ICS treatment in children, including Infants, are mild or very sporadic. However, it is important to bear in mind that a small proportion of asthmatic children may have a certain idiosyncrasy which makes them especially sensitive to ICS. STUDY DESIGN: Eleven cytokines from 128 very low gestational age Infants were analyzed from cord blood and from plasma at ages 1 day and 7 days after birth. The diagnosis of CA was based on histology of the placenta, fetal membranes, and umbilical cord. Neonatal risk factors were recorded. RESULTS: In the 48 Infants born with CA, high concentrations of inflammatory cytokines in cord blood decreased during the first postnatal day. Inflammatory cytokines in cord blood was associated with the severity of CA. At 1 day after birth, the concentration of interleukin (IL)-8 predicted the risk of BPD. For the 75 Infants born without CA, cytokine concentrations increased after birth. For the 128 Infants born with or without CA, at 1 day after birth, the concentrations of IL-8, granulocyte colony-stimulating factor, and anti-inflammatory IL-10 were associated with the risk of BPD, after adjustment for the duration of gestation and severity of respiratory distress during the first day. I am trying to book a ticket to Far East ex Scandinavia with SQ for two adults and a lap infant in First. The TA had done the res and had asked SQ CPH for a fare and tells me that a lap infant pays 100% of full adult fare ex Scandinavia:mad::mad: SQ is the best in terms of flight timing so we decided to choose SQ over TG (which is 20% cheaper). I thought the TA must have made some mistake, so contacted SQ in CPH. They tell me that there are no child or infant discounts ex Scandinavia and lap Infants and chilren pay 100%. Find it hard to believe. It seems that ex UK and Germany lap Infants pay 10% which is what most airlines charge. Some charge 10% of adult fare at time of ticketing, some charge 10% of full fare- but 100% of full F fare ?? I am no way going to pay USD 10,000+ for a six month old infant who can not sit on its seat even if it had one!! I find it hard to believe that SQ has different lap infant policy in different markets. Who do I check with as SQ CPH seems to be clueless- my TA has given up on them and is telling me to drop SQ and fly TG or EK and save lots of money.
