J Pak Med Assoc. 2008 Aug;58(8):432-6 Authors: Taj F, Tanwir M, Aly Z, Khowajah AA, Tariq A, Syed FK, Waqar F, Shahzada K OBJECTIVE: To elucidate predictors of non-adherence among psychiatric patients presenting at a tertiary Care Hospital of Pakistan, for follow-up with consultant psychiatrist. METHODS: A convenient sampleof psychiatric patients from Aga Khan University Hospital was enrolled between April and May, 2005. An interviewer assisted, standardized questionnaire was used for data collection. Patients with cognitive deficit or psychosis and those presenting for the first time were not included in the study. RESULTS: Out of 128 patients, those with co-morbidity (32.81%) were less adherent than those without comorbidity (p-value:0.002). Adherence among depressed was 61.53%; psychotic was 58.82%; bipolar disorder was 73.91%. Reasons for non-adherence included sedation (30%), medication cost (22%), forgot to take medication (36%); and inability of the physicians to explain timing and dose (92%) or benefit of medication (76%). CONCLUSIONS: Non-adherence is a common and important issue. Treatment cost and co-morbidity should be reviewed in order to keep the medication regime affordable and comprehensible. Changes in the rates of antimicrobial resistance among clinical isolates of Pseudomonas aeruginosa between 2002 and 2004 in a tertiary-Care teaching Hospital in Turkey. New Microbiol. 2008 Jul;31(3):351-5 Authors: Işeri L, Bayraktar MR Pseudomonas aeruginosa is an important opportunistic pathogen usually resistant to most antimicrobials. We present changes in the resistance pattern of P. aeruginosa to amikacin (AK) and ciprofloxacin (CIP) between January 2002 and June 2004. The physicians of each unit were given information on antibiotic resistance rates of P. aeruginosa isolated from ward patients at regular intervals. The antibiotic resistance of 161 P. aeruginosa isolates isolated from intensive Care units (ICUs) and non-ICUs were tested by disk diffusion method, and the results were interpreted according to the guidelines of National Committee for Clinical Laboratory Standards. Thirty-five percent of all the P. aeruginosa isolates were resistant to AK in 2002, 18% in 2003, and 20% in 2004. The CIP resistance rates were 4% in 2002, 26% in 2003, and 20% in 2004. In that period, resistance to AK decreased, whereas resistance to CIP increased. The usage rate of AK in 2002 was 32%, which fell to 26% in 2003 (p < 0.05). METHODS: A secondary analysis of the ED component of the 1997-2000 National Hospital Ambulatory Medical Care Survey was performed. The dependent variable was the mode of arrival to the ED (EMS vs. not EMS), and independent variables were grouped into four domains: demographic, clinical, system, and service characteristics. Bivariate analyses and multivariate logistic regression analyses were conducted. RESULTS: There were 110.9 million ED visits by children aged<19 years between 1997 and 2000. Consultants who reviewed a troubled state mental Hospital in Goldsboro have issued a report that describes a dysfunctional institution with managers too detached to effectively solve problems. The Compass Group, an Ohio-based health-Care consulting firm, was hired to assess conditions at Cherry Hospital last month after a patient death and beatings spurred federal regulators to cut off Medicare and Medicaid funds. The report, completed last week and released by the state Tuesday, says workers at Cherry are distrustful of their leaders. It went on to say they are often hesitant to report incidents of patient neglect or abuse for fear of retaliation and doubt it will do any good. Top managers, including Hospital director Jack St. Clair, were said to communicate poorly with those they supervise, often sending mass e-mails rather than talking face-to-face. They rarely visit wards where patients are cared for, the report said. Though the report says the Hospital's leadership is deficient, the consultants advised against firing them. “These leaders have not demonstrated that they are collectively up to the challenge of managing organizational change,” the report says. The staff is so demoralized by recent media reports about the Hospital's problems, employees often remove their name badges when leaving the Hospital so people won't know where they work, the report said.
