As a result, significant advances have been made in the study of HCV infection in Patients with end-stage kidney disease. Patients on hemodialysis have a higher incidence and prevalence of HCV infection than the general population. In addition, HCV infection affects adversely survival among Patients with end-stage kidney disease. Risk factors for HCV infection in Dialysis Patients include number of blood transfusions, duration of hemodialysis, mode of Dialysis, prevalence of HCV infection in the Dialysis unit, previous organ transplantation, intravenous drug use, male gender, older age and nosocomial transmission of HCV in hemodialysis units that can occur due to breakdown in standard infection control practices, physical proximity to an infected patient, cross-infection through Dialysis machines, disrupted integrity of dialyzer membrane or dialyzer reprocessing. We retrospectively studied pretransplant cardiac evaluation (CE) practices in a national sample of renal allograft recipients. DESIGN, SETTING, PARTICIPANTS, & MEASUREMENTS: The USRDS data for Medicare beneficiaries transplanted in 1991 to 2004 with Part A&B benefits from Dialysis initiation through transplantation were examined. Clinical traits defining "high" expected IHD risk were defined as diabetes, prior IHD, or > or = 2 other coronary risk factors. Pretransplant CE were identified by billing claims for noninvasive stress tests and angiography. Patients were quantified with claims for coronary revascularization procedures between CE and transplant. Post-transplant acute myocardial infarction (AMI) events were abstracted from claims and death records. RESULTS: Among 27,786 eligible Patients, 46.3% underwent CE before transplantation. METHODS: We used United States Renal Data System data to analyse the outcomes of non-fatal myocardial infarction (MI) and cardiac death in incident Dialysis Patients from years 1997 through 2001 (n = 214 890). The presence of established coronary artery disease was determined from the Medical Evidence form, non-fatal MI events were determined from Medicare claims' data and cause of death was determined from the Death Notification form. Multivariable analyses were performed employing Cox proportional hazards models using demographics, co-morbidities, laboratory variables, prior erythropoietin use, body mass index and type of Dialysis. RESULTS: In Patients with prior coronary disease as compared to those without, the adjusted relative risk of non-fatal MI was 1.57 (95% CI, 1.5-1.65) and cardiac death was 1.16 (95% CI, 1.14-1.18). The aim of our study was to assess whether AGE accumulation in Dialysis Patients is related to the presence of diastolic dysfunction. METHODS AND RESULTS: Data were analyzed from 43 Dialysis Patients, age 58 +/- 15 years, of whom 65% were male. Diastolic function was assessed using tissue velocity imaging (TVI) on echocardiography. Tissue AGE accumulation was measured using a validated skin-autofluorescence (skin-AF) reader. Plasma N(epsilon)-(carboxymethyl)lysine (CML) and N(epsilon)-(carboxyethyl)lysine (CEL) were measured by stable-isotope-dilution tandem mass spectrometry. DESIGN: We measured serum 25(OH)D concentrations in 230 prevalent peritoneal Dialysis Patients and then followed these Patients prospectively for 3 y or until death. RESULTS: Serum 25(OH)D was deficient or insufficient (ie, <75 nmol/L) in 87% of the Patients. Adjusting for clinical and demographic factors, every 1-unit increase in log-transformed serum 25(OH)D was associated with a 44% reduction in the hazard of fatal or nonfatal cardiovascular events (95% CI: 0.35, 0.91; P = 0.018). However, the association was gradually lost when additional adjustment was made in a stepwise fashion for residual glomerular filtration rate (P = 0.078) and echocardiographic measures (P = 0.39).
