Arch Intern Med. 2008 Jul 14;168(13):1404-8 Authors: Young HN, Bell RA, Epstein RM, Feldman MD, Kravitz RL BACKGROUND: Although shared decision making (SDM) has been reported to facilitate quality care, few studies have explored the extent to which SDM is implemented in primary care and factors that influence its application. This study assesses the extent to which physicians enact SDM behaviors and describes factors associated with physicians' SDM behaviors within the context of Depression care. METHODS: In a secondary analysis of data from a randomized experiment, we coded 287 audiorecorded interactions between physicians and standardized patients (SPs) using the Observing Patient Involvement (OPTION) system to assess physician SDM behaviors. We performed a series of generalized linear mixed model analyses to examine physician and patient characteristics associated with SDM behavior. RESULTS: The mean (SD) OPTION score was 11.4 (3.3) of 48 possible points. Older physicians (partial correlation coefficient = -0.29; beta = -0.09; P < .01) and physicians who practiced in a health maintenance organization setting (beta = -1.60; P < .01) performed fewer SDM behaviors. Longer visit duration was associated with more SDM behaviors (partial correlation coefficient = 0.31; beta = 0.08; P < .01). In addition, physicians enacted more SDM behaviors with SPs who made ... Related Articles Depression, quality of life and breast cancer: a review of the literature. Breast Cancer Res Treat. 2008 Jul;110(1):9-17 Authors: Reich M, Lesur A, Perdrizet-Chevallier C Depression is misdiagnosed and undertreated among breast cancer population. Risk factors for Depression in the 5 years after diagnosis are related more to the patient rather than to the disease or its treatment. The breast cancer stage (early and advanced) is not statistically significant in terms of rates of psychosocial distress except for recurrence. Risk factors of Depression might impair quality of life such as fatigue, past history or recent episode of Depression after the onset of cancer, cognitive attitudes of helplessness/hopelessness, resignation. Body image impairment from mastectomy and sexuality aftermath generates higher rates of mood disorders. The link between increased risk of breast cancer and Depression is controversial among the literature. Some studies suggest a protective factor, others find a relation between stress, immunity and cancer occurrence or even mortality. Breast cancer survivors report a higher prevalence of mild to moderate Depression with a lower quality of life in all areas except for family functioning. Treatment of Depression in breast cancer women improves their quality of life and may increase longevity. Antidepressant medications remain the cornerstone of Depression treatment. Related Articles Outcomes and predictors of late-life Depression trajectories in older primary care patients. Am J Geriatr Psychiatry. 2008 May;16(5):406-15 Authors: Cui X, Lyness JM, Tang W, Tu X, Conwell Y OBJECTIVES: The naturalistic outcomes of Depression in older primary care patients have been poorly characterized. The authors sought to identify depressive trajectories over 2 years and to examine specified outcome predictors. DESIGN: Two-year observational cohort study. SETTING: University-based and independent practice primary care practices in greater Rochester. PARTICIPANTS: All patients aged >65 years presenting for care on selected recruitment days were eligible to participate. Of 392 subjects enrolled, 316 (80.6%) completed study measures over a 2-year follow-up. MEASUREMENTS: Depression trajectories were derived by applying longitudinal cluster analysis to weekly Depression status from the Longitudinal Interval Follow-up Evaluation. RESULTS: The authors identified six distinct trajectory clusters that followed clinically intuitive patterns. Although subjects initially nondepressed or in the subsyndromal to minor Depression range had a range of possible outcomes over 2 years, the cluster initially near the major Depression level remained at that level over time. Consistent predictors of Depression trajectory were baseline depressive symptom severity, medical burden, and psychiatric functional status; for some clusters, previ ...
