Contemp Nurse. 2008 May;29(1):23-31 Authors: Hare M, Wynaden D, McGowan S, Landsborough I, Speed G Abstract Delirium is a frequent complication of hospital admission, especially among the elderly. It can have serious consequences in terms of morbidity, mortality and decreased quality of life. Nevertheless, an extensive literature review found that it is poorly recognised and poorly managed by medical and nursing staff. Although some researchers have found that education programs for nurses can improve outcomes for patients with Delirium, no research assessing nurses' knowledge was found. The objective of this research was to determine nurses' level of knowledge regarding Delirium and its risk factors. A questionnaire survey sent to nurses at a teaching hospital found that nurse's knowledge of Delirium was generally inadequate, although one ward which had had in-service education attained better results. It is recommended that cognitive assessment in general and Delirium in particular be incorporated into nursing education. We retrospectively examined the incidence of postoperative Delirium and the probable risk factors in patients undergoing spinal surgery in our hospital, with the same aged non-Delirium patients as controls, over a 6-month period. Studies about postoperative Delirium were reviewed and referenced for variable factors collecting in our study. T tests, chi (2) test and logistic regression analysis were performed to evaluate the various factors related to postoperative Delirium. A total of 18 patients (3.3%), all of them were aged 54 years or older, had postoperative Delirium after surgery. Patients without postoperative Delirium aged 54 years or older served as the control group. The percentage of patients older than 65 years (P = 0.003), with comorbid diseases such as diabetes mellitus (P = 0.042) or central nervous system disorders (P = 0.013), with a surgical history (P = 0.028) in Delirium group was larger than the control group. Tryptophan is an amino acid precursor to the mood-stabilizing neurotransmitters serotonin and melatonin. We hypothesized that tryptophan levels are lower in elderly subjects who develop postoperative Delirium. METHODS: A prospective observational study was performed. Subjects older than 50 years undergoing surgery with an anticipated postoperative intensive care unit admission were recruited. Postoperative Delirium assessment occurred daily using the Confusion Assessment Method-intensive care unit. Peripheral serum tryptophan levels were measured 2 days after surgery. RESULTS: Forty-nine subjects (46 men) were enrolled, with an average age of 64 +/- 7 years. The incidence of Delirium was 43% (21 of 49). The average duration of Delirium was 2.9 +/- 3.0 days. Tryptophan levels were lower in the subjects who developed Delirium (29.9 +/- 13.3 vs 48.5 +/- 19.8 mug/mL; P = .001). We sought to determine whether the use of statin medications was associated with a higher risk of postoperative Delirium than other medications that do not alter microvascular autoregulation. METHODS: We conducted a retrospective cohort analysis of 284 158 consecutive patients in Ontario aged 65 years and older who were admitted for elective surgery. We identified exposure to statins from outpatient pharmacy records before admission. We identified Delirium by examining hospital records after surgery. RESULTS: About 7% (n = 19 501) of the patients were taking statins. Overall, 3195 patients experienced postoperative Delirium; the rate was significantly higher among patients taking statins (14 per 1000) than among those not taking statins (11 per 1000) (odds ratio [OR] 1.30, 95% confidence interval [CI] 1.15-1.47, p < 0.001). The increased risk of postoperative Delirium persisted after we adjusted for multiple demographic, medical and surgical factors (OR 1.28, 95% CI 1.12-1.46) and exceeded the increased risk of Delirium associated with prolonging surgery by 30 minutes (OR 1.20, 95% CI 1.19-1.21).
