Related Articles Systemic associations and prevalence of exfoliation syndrome in patients scheduled for Cataract Surgery. Eur J Ophthalmol. 2008 Jul-Aug;18(4):551-5 Authors: Sekeroglu MA, Bozkurt B, Irkec M, Ustunel S, Orhan M, Saracbasi O PURPOSE: To study the frequency of exfoliation syndrome (XFS) and its association with intraocular pressure (IOP) and systemic diseases in patients with age-related Cataract scheduled for Surgery. METHODS: All 1480 Cataract patients had a comprehensive systemic and eye examination, including slit-lamp biomicroscopy before and after mydriasis, IOP measurement, gonioscopy, and fundus examination. RESULTS: The percentage of XFS among patients scheduled for Cataract Surgery was 16.4, with no gender difference (p=0.833). The mean age of XFS patients (74.3+/-7.0 years) was significantly higher when compared to the ones without XFS (66.5+/-10.9 years) (p<0.001). XFS was unilateral in 41.3% of the subjects and bilateral in 58.7%. The number of subjects with XFS increased significantly with aging (p<0.001) (OR=1.093, 95% CI=1.073-1.14) (p<0.001). The most common type of Cataract was nuclear in the XFS group (33.5%) and 24.6% of patients with mature Cataract had XFS. In the XFS group, 11.2% of the subjects had glaucoma, which was statistically higher than the non-XFS group (4.6%) (p<0.001) (OR=2.67, 95% CI=1.65-4.32). Eighty-four patients had glaucoma and 27 of them (32.1%) had exfoliative glaucoma. The only s ... Source: www.ncbi.nlm.nih.gov --- 18 days ago
Efficacy of topical plus intracameral anesthesia for Cataract Surgery in high myopia: Randomized controlled trial. J Cataract Refract Surg. 2008 Oct;34(10):1664-8 Authors: Lofoco G, Ciucci F, Bardocci A, Quercioli P, De Gaetano C, Ghirelli G, Perdicaro S, Lomoriello DS, Cacciamani A PURPOSE: To assess the efficacy of intracameral lidocaine supplementation of topical anesthesia during Cataract Surgery in eyes with high myopia. SETTING: Department of Ophthalmology, Ospedale San Pietro-Fatebenefratelli, Rome, Italy. METHODS: This prospective double-blind study comprised 120 highly myopic eyes with an axial length (AL) greater than 26.0 mm scheduled for routine Cataract Surgery. Cases were divided into 2 groups of 60 eyes each. One group received a placebo of balanced salt solution (BSS) (control group) and the other group, a supplement of 0.1 mL preservative-free lidocaine hydrochloride 1% injected in the capsular bag during hydrodissection (lidocaine group). Intraoperative pain was assessed by recording spontaneous patient reports of sensation of pain or ocular discomfort during 3 surgical stages: phaco tip insertion, irrigation/aspiration (I/A) system insertion for cortical aspiration, I/A system insertion for ophthalmic viscosurgical device removal after intraocular lens implantation. Postoperative pain was assessed on a visual analog scale (range 0 to 10). Data were compared by chi-square and Mann-Whitney U tests. RESULTS: The overal ... Related Articles Emergence of diplopia and oscillopsia due to Heimann-Bielschowsky phenomenon after Cataract Surgery. Br J Ophthalmol. 2008 Oct;92(10):1402 Authors: Jeong SH, Oh YM, Hwang JM, Kim JS The Heimann-Bielschowsky phenomenon (HBP) refers to coarse vertical oscillation of the eye with impaired vision. The ocular movements are strictly monocular, occurring only in the eye with amblyopia. The vertical oscillation is of equal velocity in both vertical directions, or may sometimes be greater in the downward than upward direction. HBP develops several years after loss of vision. It can be differentiated from dissociated nystagmus in spasmus nutans, congenital nystagmus and internuclear ophthalmoplegia based on the strict unilaterality, vertical direction and low frequency. Previously, only a few reports described the development of oscillopsia due to HBP after Cataract Surgery, which resolved spontaneously or responded to gabapentin. However, visual impairments due to diplopia or oscillopsia from HBP after Cataract Surgery have received little attention. We report a man who developed persistent vertical diplopia and oscillopsia due to HBP after a Cataract operation, which markedly impaired his vision. PMID: 18815421 [PubMed - in process] ... Br J Ophthalmol. 2008 Aug 29; Authors: Denoyer A, Denoyer L, Marotte D, Georget M, Pisella PJ AIM: To compare the optical outcomes of biaxial microincision Cataract Surgery (B-MICS) versus coaxial small-incision Cataract Surgery (C-SICS). METHODS: Thirty patients (sixty eyes) with bilateral age-related Cataract underwent B-MICS through a 1.7-mm clear corneal incision (CCI) in one eye and C-SICS through a 2.8-mm CCI in the other eye. Every eye was implanted with a micro incision aspheric aberration-free IOL (Akreos MI60 AO, Bausch and Lomb, USA). Preoperative corneal aberrations, and 3-month postoperative corneal and total ocular aberrations were calculated and compared. RESULTS: 3-month postoperative corneal root mean square (RMS) of 3rd- to 6th-order aberrations (p<0.001), 3rd-order trefoil (p=0.001), trefoil-like (p<0.001), and 3rd order RMS (p<0.001) were lower in B-MICS group than in C-SICS group. Surgically-induced corneal trefoil was reduced in B-MICS eyes (p=0.001). Total ocular high-order RMS (p=0.002), 3rd-trefoil (p<0.001), trefoil-like (p<0.001) and 5th-order RMS (p<0.001) were lower in B-MICS than in C-SICS eyes. Corneal aberrations were positively correlated with ocular ones for RMS of 3rd- to 6th-order aberrations, 3rd-trefoil and spherical-like aberrati ... Source: www.ncbi.nlm.nih.gov --- 18 days ago
Accuracy of preoperative biometry in vitrectomy combined with Cataract Surgery for patients with epiretinal membranes and macular holes Results of a prospective controlled clinical trial. J Cataract Refract Surg. 2008 Oct;34(10):1754-1760 Authors: Falkner-Radler CI, Benesch T, Binder S PURPOSE: To evaluate how adding vitrectomy to Cataract Surgery affects the accuracy of preoperative biometry and postoperative refractive outcomes. SETTING: Department of Ophthalmology, Ludwig Boltzmann Institute of Retinology and Biomicroscopic Lasersurgery, Rudolf Foundation Clinic, Vienna, Austria. METHODS: This study comprised 40 patients with vitreoretinal pathology and coexisting significant Cataract (study group) and 40 patients with significant Cataract only (control group). The main outcome measure was intraocular lens (IOL) power prediction error. Secondary outcome measures were spherical equivalent, anterior chamber depth, axial length, keratometry values, and intraocular pressure. A multivariate regression analysis was performed. RESULTS: There was a significant difference in prediction error between the study group and control group (P<.05). The addition of vitrectomy was associated with induced myopia of approximately -0.4 diopter, more so in patients who had epiretinal membrane removal than in patients with macular hole Surgery (P = .04). There was no significant difference in prediction error between the 3 IOLs used (P = .2). CONCLUSION: ... Source: www.ncbi.nlm.nih.gov --- 37 days ago
