[Subcutaneous mastectomy with reduction mammaplasty--challenge for the surgeon and possible alternative for Breast Cancer Patients] Khirurgiia (Sofiia). 2006;(1):17-20 Authors: INTRODUCTION: Breast Cancer surgery with Breast removal is associated with critical decrease in women quality of life. Oncoplastic surgery offer different techniques for removal of the tumor and remodelating the Breast to achieve acceptable end results--oncological and aesthetical ones. MATERIAL AND METHODS: We present the clinical case of M.G. M -72 year old, St.IIIA--Breast Cancer patient, who refuses removal of the Breast and any chemotherapy. RESULTS: We perform subcutaneous mastectomy through inverted T - incision, including the areolo - mammilary complex in mastectomy specimen; L1, L2, L3 - lymphadenectomy through lateral skin flap. Skin - island flap was designed, based on superior flap for areolar replacement. Areolar reconstruction and reduction for symmetry were planed as a second step procedure because of the patient informed consent. CONCLUSIONS: 1. Subcutaneous mastectomy with reduction mammaplasty is an acceptable and possible operative way for Breast Cancer patient. 2. Preoperative planning is critical for best end results. 3. All precautions must by take for primary wound healing to achieve best time between operation and postoperative irradiation. 4. Recently published prospective studies question this hypothesis, but it is still unclear whether Cancer Patients should regard cognitive problems as a potential risk when receiving chemotherapy.Methods: In the present study we examine whether Cancer Patients (n=34) receiving chemotherapy differed in cognitive changes during treatment compared with cardiac Patients (n=12) and healthy controls (n=12) tested at 3-4 months interval.Results: Our results showed no differences with respect to changes in cognitive performance over time between Cancer Patients in chemotherapy, cardiac Patients, and healthy controls. In addition, the number of individuals showing reliable decline or improvement on cognitive tests did not differ between groups.Conclusion: Taken together, our results do not support a hypothesis of cognitive side-effects of standard-dose chemotherapy in Breast Cancer Patients. Mean volume of existed tissue was 43 cm3. Margins were adequate for 12 Patients and the final 2 Patients in the serie had to undergo mastectomy for involved margins. At one year, cosmetic results were graded as good by both the surgeon and the Patients. Mammograms were satisfactory except for one case of partial liposclerosis of the flap.
