Am J Surg. 2008 Aug 28; Authors: Lee HK, Han HS, Min SK BACKGROUND: Severe cholecystitis occurs more frequently in males, and the cause of this discrepancy is unknown. METHODS: A retrospective study was performed on 910 consecutive patients who underwent laparoscopic cholecystectomy for cholecystitis. The patients were classified according to Body Mass Index (BMI) as obese (n = 354, BMI >/=25 kg/m(2)) and nonobese (n = 556; BMI <25). RESULTS: In males, there was a significant negative correlation between the BMI and the severity of cholecystitis; the proportion of complicated acute cholecystitis was higher in the nonobese patients (21.5%) compared with the obese patients (8.1%) (P = .007) but not for the females (P = .80). A BMI <25 (odds ratio [OR] = 1.92, P = .01), advanced age (OR = 2.52, P < .001), male sex (OR = 1.74, P = .022), and leukocytosis (OR = 1.92, P = .024) were independent predictors for the development of complicated acute cholecystitis. CONCLUSION: There is a negative association between BMI and the inflammation severity of cholecystitis in males, which resulted in a higher incidence of severe cholecystitis in the nonobese male patients. Our goal was to analyze the relationship of Body Mass Index (BMI), regular sports participation, TV viewing, and diet with current wheezing and asthma. METHODS: We investigated 20,016 children, aged 6-7 years, who were enrolled in a population-based study. Parents completed standardized questionnaires. Logistic regression was used to estimate odds ratios (ORs) and 95% confidence intervals (CIs), while adjusting for several confounders and simultaneously considering BMI, regular sports activity, TV viewing and selected dietary items. RESULTS: A total of 1575 children (7.9%) reported current wheezing and 1343 (6.7%) reported current asthma. METHODS: One hundred sixteen HD patients were stratified into 4 groups according to the BMI: underweight (<18.5), normal weight (18.5-24.9), overweight (25.0-29.9) and obese (> or =30). Blood samples were obtained before the HD session to measure serum albumin, high-sensitivity C-reactive protein, fibrinogen, ferritin, total cholesterol, LDL cholesterol, HDL cholesterol, apolipoprotein A-I and apolipoprotein B-100, apolipoprotein B (apoB) to apolipoprotein A (apoA) ratio and Lp(a) lipoprotein. RESULTS: There were 3 underweight (excluded from the analysis), 58 normal weight, 35 overweight and 20 obese patients. Their mean age was 62.1 +/- 14.1 years. There were 68 men and 45 women. STUDY DESIGN: The 1999-2004 National Health and Nutrition Examination Surveys (NHANES) cross-sectional data for 12- to 19-year-old adolescents (n = 2581) was analyzed. Main outcome measures were >/=3 age-adjusted CVD RFs (high- and low-density lipoprotein cholesterol level, triglyceride level, glucose level, insulin level, and systolic and diastolic blood pressure). The presence of >/=3 RFs was predicted from age- and sex-adjusted BMI and WC values with receiver operating characteristics analyses. RESULTS: The proportion of adolescents at risk for >/=3 RFs ranged from 17% to 19%. Both BMI and WC had good diagnostic accuracy, ranging from 0.73 to 0.83, and good sensitivity and specificity, ranging from 0.68 to 0.77. The BMI cutoff points ranged from 19.5 to 25.0 kg/m(2) for boys and from 19.4 to 27.0 kg/m(2) for girls, and WC cutoff points ranged from 66.8 to 87.5 cm for boys and from 71.5 to 87.2 cm for girls. BMI was available for 150 patients (75%), who comprised our study group. We obtained direct and subcomponent costs (room and board, laboratory, pharmacy, radiology, operating room, surgical supplies, anesthesia, and recovery room). We divided patients into four BMI categories: normal weight (BMI < 25), overweight (25 </= BMI < 30), obese (30 </= BMI < 40), and morbidly obese (BMI >/= 40). We compared groups with regard to baseline characteristics, intraoperative parameters, stone-free and complication rates, and hospital length of stay. RESULTS: Mean stone size and proportion of patients with staghorn, multiple, and bilateral calculi were similar among groups. Multivariate regression analysis was applied to estimate the cost for all drugs and the 'top ten' drugs at each BMI point. This allowed the total and attributable prescribing costs to be estimated at any BMI. Weight loss outcomes achieved in a weight management programme (Counterweight) were used to model potential effects of weight change on drug costs. Okay, first we had the Centers for Disease Control and Prevention study that found last year 27 percent of U.S. adults age 20 and older were obese – as in 30 pounds above their ideal weight. That was up from 24 percent in 2005. Which was up from the 19 percent of obese adults in 1997. Once you add in the rest of the Americans considered overweight, the statistical scale jumps to more than 65 percent of Americans. Got that. Then a few weeks later, another study is released predicting that at the rate the U.S. is headed, 86 percent of adults would be overweight and a whopping 51 percent obese by 2030. Now comes what I call the Monster Thickburger (anyone else remember that 1,420-calorie Hardee’s sandwich) of all government-funded studies. It anticipates that every single solitary American adult could be overweight in 40 years (that is 2048) based on the trends over the last 30 years. These researchers also anticipate that by 2034 all black women and more than 90 percent of Mexican American men will overweight. All this research rests on the BIG (Mac) assumption that the trends of recent decades will march on unabated, researchers said.
