Related Articles Hopelessness across phases of Bipolar I or II Disorder: A prospective study. J Affect Disord. 2008 Aug 2; Authors: Valtonen HM, Suominen K, Haukka J, Mantere O, Arvilommi P, Leppämäki S, Isometsä ET BACKGROUND: Hopelessness, a key risk factor for suicidal behaviour overall, has been studied little among Bipolar Disorder (BD) patients. For purposes of prevention, it is important to know whether it is predominantly a patient's permanent trait or merely reflects the highly variable illness states. We investigated the degree to which hopelessness is trait- or state-related during the course of BD. METHODS: The Jorvi Bipolar Study (JoBS) is a naturalistic prospective study representing psychiatric in- and outpatients with DSM-IV BD I and II. Repeated measurements with the Beck Hopelessness Scale of 188 patients at baseline, 6 months and 18 months were analysed using a linear regression model with general estimation equations. Factors covarying with hopelessness during follow-up were investigated. RESULTS: Levels of hopelessness varied markedly between illness phases, being highest in depressive and mixed phases, and lowest in euthymia, hypomania or mania. Hopelessness was independently associated with concurrent severity of depression (estimate 0.231, p<0.001), anxiety (0.105, p<0.001), fewer manic symptoms (-0.096, p=0.001) and comorbid personality Disorder (1.741, p=0.001). However, the strongest predictor of degree of ho ... Source: www.ncbi.nlm.nih.gov --- 3 days ago
Related Articles [Antidepressants in Bipolar Disorder: beware of risks!] Rev Med Liege. 2008 May-Jun;63(5-6):391-5 Authors: Pitchot W, Scantamburlo G, Ansseau M The value of antidepressants in Bipolar Disorder is the subject of significant controversy. The prescription of antidepressants remains frequent in clinical practice despite international guidelines recommending serious carefulness in their use. Caution should be exerted before the initiation of a treatment for an episode of major depression in a Bipolar patient. The prescription of antidepressants might be envisaged in case of therapeutic failure with a thymostabilizing agent, and always in association with a preventive therapy. Br J Psychiatry. 2008 Aug;193(2):126-9 Authors: Wang F, Jackowski M, Kalmar JH, Chepenik LG, Tie K, Qiu M, Gong G, Pittman BP, Jones MM, Shah MP, Spencer L, Papademetris X, Constable RT, Blumberg HP BACKGROUND: Convergent evidence implicates white matter abnormalities in Bipolar Disorder. The cingulum is an important candidate structure for study in Bipolar Disorder as it provides substantial white matter connections within the corticolimbic neural system that subserves emotional regulation involved in the Disorder. AIMS: To test the hypothesis that Bipolar Disorder is associated with abnormal white matter integrity in the cingulum. METHOD: Fractional anisotropy in the anterior and posterior cingulum was compared between 42 participants with Bipolar Disorder and 42 healthy participants using diffusion tensor imaging. RESULTS: Fractional anisotropy was significantly decreased in the anterior cingulum in the Bipolar Disorder group compared with the healthy group (P=0.003); however, fractional anisotropy in the posterior cingulum did not differ significantly between groups. CONCLUSIONS: Our findings demonstrate abnormalities in the structural integrity of the anterior cingulum in Bipolar Disorder. They extend evidence that supports involvement of the neural system comprising the anterior cingulate cortex and its corticolim ... Source: www.ncbi.nlm.nih.gov --- 5 days ago

Psychopharmacol Bull. 2008;41(2):89-92 Authors: Trakada G, Steiropoulos P, Bouros D We present a case of a 43-year-old woman with Bipolar Disorder who was diagnosed with sleep apnea-hypopnea syndrome (SAHS). After 10 days of treatment with continuous positive airways pressure (CPAP) during sleep, the patient presented a mood switch, from depression to mania and was admitted to the hospital.Withdrawal of CPAP and appropriate therapy controlled the episode. Related Articles Criteria for Defining Symptomatic and Sustained Remission in Bipolar I Disorder: a Post-Hoc Analysis of a 26-Week Aripiprazole Study (Study CN138-010). Psychopharmacol Bull. 2008;41(2):12-23 Authors: Masand PS, Eudicone J, Pikalov A, McQuade RD, Marcus RN, Vester-Blokland E, Carlson BX Objective: Remission is a key goal after treating an acute episode of Bipolar I Disorder, but greater understanding is needed of the correlation between attaining remission at a specific time point and maintaining sustained remission. This post-hoc analysis assessed symptomatic point remission and sustained remission according to either a standard criterion (YMRS = 12) or a set of more rigorous criteria (YMRS = 7, MADRS = 10, and CGI-I = 1) using data from a 26-week, randomized, double-blind, placebocontrolled study with the atypical antipsychotic aripiprazole in patients with Bipolar I Disorder. Methods: Following >/= 6 consecutive weeks' stabilization with open-label aripiprazole, 161 patients were randomized (1:1) to aripiprazole or placebo for up to 26 weeks. Symptomatic remission rates were determined at Weeks 8, 16, and 26; sustained remission rates were determined at each visit up until Weeks 8, 16, and 26, including a requirement to maintain remission for >/= 8 consecutive weeks (frequency counts, LOCF analysis). Results: Compared with the standard criterion (YMRS = 12), symptomatic and sustained remission criteria were f ... Source: downloads.zdnet.com --- 16 days ago
