More recently, off-pump transplantation has become the standard of care. The efficacy of aprotinin use in this population has yet to be definitively examined. We completed a retrospective review of all adult OP-BOLTs performed between January 2000 and January 2006 at a single university center (n=215). Aprotinin use was determined by the attending anesthesiologist or surgeon. It was administered at the time of induction. The primary outcome was total blood products utilized in terms of units transfused during post-operative days 0, 1 and 2. One-hundred and one patients received aprotinin and 114 did not. An overall analysis of all of the patients in this study demonstrated a trend towards statistical significance for reduced total blood product transfusion for the aprotinin group compared to the non-aprotinin group (p=0.13). A subgroup analysis was performed in relation to each diagnosis. We report the first successful case of Bilateral lung transplantation for the treatment of end-stage pulmonary lymphangiomatosis. A successful outcome was achieved with continued survival beyond 4 years posttransplant and stable lung function. The primary obstacles to significant gains in pulmonary function were thoracic, skeletal and abdominal lymphangiomatosis, which led to pulmonary restriction. Our report demonstrates that pulmonary lymphangiomatosis should be included among those diseases for which lung transplantation is considered potentially beneficial treatment but also emphasizes the importance of screening patients carefully for chest wall and abdominal lymphangiomas that may impede recovery. Most inner ear abnormalities occur in syndromes and are associated with hearing loss. Unilateral or Bilateral single aplasia of one semicircular canal does not usually result in vertigo, but these become clinically important if there are clinical complaints of vertigo. Computed tomography imaging and high resolution magnetic resonance imaging may reveal inner ear abnormalities. The case is presented here of a 46-year-old male with a 10-year history of recurrent positional vertigo with strong onset when changing position to the left side. Magnetic resonance imaging of the inner ear showed a Bilateral posterior semicircular canal aplasia as well as an enlarged vestibule on both sides. Dix-Hallpike positional manoeuvre revealed a positional nystagmus in the left head-hanging position of short duration and latency of a few seconds. When rising, vertigo occurred, but no nystagmus was visible. The fast phase of the nystagmus was mainly vertical down-beating with a slight torsional component to the uppermost ear. CT of the chest and abdomen showed Bilateral pleural effusions with a thickened pleura, nodular infiltrations in both lungs, enlarged intraabdominal lymph nodes and splenomegaly. Pleuroscopy (medical thoracoscopy) on the left side revealed dense tumorous nodules mainly on the posterior chest wall pleura, but also on the diaphragm and the lung. Biopsies taken from the chest wall pleura revealed extramedullary hematopoiesis (EMH) with abnormal megakaryocytes as well as myeloid and erythroid precursors. After unsuccessful tetracycline pleurodesis, talcum slurry was instilled via the chest tube without recurrence of the pleural effusion. Furthermore, treatment with hydroxyurea was started, and the disease regressed and then remained stable over the next 24 months. In conclusion, the pleuropulmonary findings were caused by EMH due to chronic idiopathic myelofibrosis. METHODS: From June 2003 through November 2007, the study group was comprised of 68 consecutive patients with benign thyroid nodules (66 women; mean age, 33.28 +/- 10.3 (range, 15-72) years). A detailed description of the surgical technique is provided. RESULTS: The mean maximum diameter of the tumor was 3.14 +/- 1.61 (range, 1-10.7) cm. Sixty-five cases were successfully resected with an endoscopic procedure, but three cases were converted to a conventional open procedure due to bleeding. Endoscopic procedures were 61 lobectomies, 2 isthmectomies, and 2 near total thyroidectomies. The mean operating time was 89.93 +/- 24.71 (range, 50-170) minutes, and the last 32 cases tended to take less time than the first 33 cases (P = 0.033). Abstract   Patients with knee osteoarthritis (OA) often present with symptoms that warrant Bilateral TKAs. There are potential benefits to operating on both knees on the same day, but the safety of simultaneous Bilateral TKAs has been questioned. To evaluate whether there were any differences in 30-day mortality between patients having simultaneous Bilateral TKAs and those having staged Bilateral TKAs, we analyzed data from the Swedish Knee Arthroplasty Register and the Swedish Cause of Death Register. We included 48,931 patients with OA having 60,062 primary TKAs during 1985 to 2004; 1139 had surgery on both knees on the same day (simultaneous Bilateral) and 3432 had surgery on both knees on two different occasions with less than 1 year between operations (staged Bilateral). The 30-day mortality after simultaneous Bilateral TKAs was 7.53 (confidence interval, 2.62–21.69) times higher than after the second of staged TKA and 3.77 (confidence interval, 2.04–6.98) times higher than after a primary unilateral TKA. Assuming the total risk for a staged procedure is twice that of a unilateral procedure, the risk of mortality within 30 days is 1.94 (confidence interval, 1.05–3.59) times higher with simultaneous than staged TKA.
