Related Articles Tenotomy Versus Tenodesis in the Management of Pathologic Lesions of the Tendon of the Long Head of the Biceps Brachii. Am J Sports Med. 2008 Sep 1; Authors: Frost A, Zafar MS, Maffulli N BACKGROUND: Primary and secondary lesions of the tendon of the long head of the Biceps brachii are common, with no clear consensus about their optimal management. HYPOTHESIS: There is no difference in outcomes of tenotomy and tenodesis for lesions of the tendon of the long head of the Biceps brachii. STUDY DESIGN: We performed a comprehensive quantitative review of the published English-language literature comparing the outcomes of tenotony and tenodesis for lesions of the tendon of the long head of the Biceps brachii. METHODS: All relevant articles in peer-reviewed journals were retrieved, and each article was scored using the Coleman Methodology Score, a highly repeatable methodology score, by 2 independent reviewers. RESULTS: Scores were predominantly low for quality of the studies, with patient number and validated outcome measures being the weakest areas. CONCLUSION: There is a lack of quality evidence to advocate one technique over the other. We emphasize the need for appropriately powered, well-conducted, randomized, controlled trials comparing the outcomes of these 2 procedures. The proximal Biceps as a pain generator and results of tenotomy. Sports Med Arthrosc. 2008 Sep;16(3):180-6 Authors: Szabó I, Boileau P, Walch G Many palliative interventions have been proposed for patients in whom rotator cuff repair is not feasible as a result of advanced fatty infiltration, definitive loss of tendons, and proximal humeral migration. The long head of the Biceps tendon has been proposed as a source of pain in patients with rotator cuff tears. This article presents a review of current concepts on the rationale for arthroscopic Biceps tenotomy or tenodesis, and evaluates the objective, subjective, and radiographic results of these palliative procedures. On the basis of different studies, it seems that isolated arthroscopic Biceps tenotomy or tenodesis is a valuable option for the treatment of rotator cuff tears in selected patients. Although it does not improve shoulder strength, tenotomy or tenodesis reduces pain and improves the functional range of motion with a high degree of patient satisfaction. This article reviews the clinical findings associated with LHB pathology, describes the operative technique of subpectoral Biceps tenodesis, and reviews the current literature related to treatment of the symptomatic LHB tendon. The miniopen subpectoral approach is technically less demanding than purely arthroscopic techniques, and offers the potential for improved pain relief without cosmetic deformity by removing most of the LHB and its associated tenosynovium. J Electromyogr Kinesiol. 2008 Aug 6; Authors: Fling BW, Christie A, Kamen G Motor unit (MU) synchronization is the simultaneous or near-simultaneous firing of two MUs which occurs more often than would be expected by chance. The present study sought to investigate the effects of exercise training, muscle group, and force level, by comparing the magnitude of synchronization in the Biceps brachii (BB) and first dorsal interosseous (FDI) muscles of untrained and strength-trained college-aged males at two force levels, 30% of maximal voluntary contraction (MVC) and 80% MVC. MU action potentials were recorded directly via an intramuscular needle electrode. The magnitude of synchronization was assessed using previously-reported synchronization indices: k', E, and CIS. Synchronization was significantly higher in the FDI than in the BB. Greater synchronization was observed in the strength-trained group with CIS, but not with E or k'. Also, synchronization was significantly greater at 80% MVC than at 30% MVC with E, but only moderately greater with CIS and there was no force difference with k'. Synchronization prevalence was found to be greater in the BB (80.1%) than in the FDI (71.5%). Thus, although the evidence is a bit equivocal, it appears that MU synchronization is greater at higher forces, and greater in strength-trained individuals ...

A diagnosis of Biceps tendinosis was made using a specific diagnostic protocol coupled with observation of Biceps tendon fraying and increased erythema on dry arthroscopy. RESULTS: Between November 2002 and August 2005, 50 patients underwent subpectoral Biceps tenodesis. Complete follow-up examinations were performed in 41 of 50 (82%). There were 16 women and 25 men (mean age, 50 years). Follow-up averaged 29 months (range, 12-49 months). I think its just me but lately, I've been noticing many Biceps tear from guys (mainly powerlifters) deadlifting. They all seem to be in the arm that is supinated which makes sense to me, but has it always been this common in deadlifting maximal weights? I understand that there are probably many reasons why a bicep can tear. However, in regards to it's involvement in the deadlift, what do you guys think the main problems are? I'm not a powerlifter. I just like deadlifting, but I've heard that some powerlifters avoid doing little to no direct Biceps exercises. If this is true, could this be one of the reasons why an injury like this can occur? Have any of you encounter this type of injury from deadlifting before and what do you think are the appropriate approaches one can take to avoid it? Thanks. ...
