Obstructive sleep apnea (OSA) is a commonly encountered comorbidity in morbidly obese patients. Sedatives, analgesics, and anesthetics alter airway tone, and airway obstruction and death have been reported in patients with OSA after minimal doses of sedatives and anesthetics, yet there is a lack of consensus regarding the care of these patients. In this study, we sought to determine whether obese patients with polysomnography-confirmed diagnosis of OSA were at significantly greater risk for postoperative hypoxemic episodes in the first 24 h after laparoscopic Bariatric Surgery than morbidly obese patients without a diagnosis of OSA. METHODS: Adult subjects (Body Mass Index, 35-75 kg/m(2)) scheduled to undergo laparoscopic Bariatric Surgery were studied. Surg Endosc. 2008 Jul 12; Authors: Raftopoulos I, Martindale C, Cronin A, Steinberg J BACKGROUND: The effect of extended post-discharge thromboprophylaxis (ETP) on venous thromboembolism (VTE) rates following Bariatric Surgery (BS) is unknown. METHODS: 308 consecutive patients who underwent BS between 2003 and 2007 and who had >1 month of follow-up were included. In-hospital-only VTE prophylaxis (group A), or extended 10-day ETP (group B) was used in 132 and 176 patients, respectively. All patients underwent bilateral lower extremity venous Doppler studies (BLEVDS) prior to discharge. Primary endpoint was the incidence of VTE within 30 days postoperatively. VTE was defined as a clinically evident deep vein thrombosis or pulmonary embolism documented by positive BLEVDS, or computed chest tomography. The primary safety endpoint was bleeding associated with >/=2 g/dL decrease in hemoglobin compared with baseline, transfusion or reoperation. RESULTS: The incidence of VTE was 1.9% (6/308); 66.6% (4/6) of cases occurred after cessation of thromboprophylaxis. There were no deaths in either group. Frustrated by limits imposed by U.S. health plans--which typically require patients to have a BMI of 40--patients are increasingly going to Mexico to get Bariatric surgeries. They're doing this even though Mexican prices for Bariatric surgical procedures are only slightly lower than those in the U.S.--and even though there are still some questions as to whether they'll get a safe, high-quality procedure. Patients say they simply can't get their procedures in this country. Part of the reasons current guidelines are so restrictive is that they're somewhat dated. Current Bariatric Surgery guidelines were based on research from the 1970s and 1980s, before the introduction of gastric banding Surgery. Unlike the more invasive, irreversible gastric bypass, gastric banding can be performed laparoscopically and can be reversed if need be. Now, Mexican providers are making hay of the fact that it's hard to get procedures approved in the U.S. Scores of websites advertise Mexican Bariatric Surgery options, with many pitching their proximity to American airports. One clinic with a site in Cancun has an affiliation with a network of U.S. centers allowing patients to get refills of the solution required to inflate the gastric band and produce a feeling of fullness. Obes Surg. 2008 Jul 10; Authors: Cubattoli L, Barneschi C, Mastrocinque E, Bonucci P, Giomarelli PP A super-obese patient who suffered from severe sleep apnea (SSA) and other comorbidities underwent insertion of a BioEnterics intragastric balloon (BIB) before Bariatric Surgery. During the night, he was victim of cardiac arrest. After cardiopulmonary resuscitation and return of spontaneous circulation, he was transferred to intensive care unit. Two hours later, he developed an unexpected symptomatic bradycardia, and BIB was removed. The patient had no further cardiac complications, but he had a poor neurological outcome. In our opinion, such a severe cardiac event was the result of several causes. The BIB induces vagal nerve activation by stretching the gastric wall. In addition, super-obese patients with sleep apnea and other comorbitities have an increased risk of potentially fatal cardiac arrhythmias especially during the night. For all these reasons, we think that these patients may benefit from further preoperative cardiac investigations and a more intensive control during the first postinsertion day.
