Int J Cardiol. 2008 Jul 4;127(2):228-32 Authors: Logeart D, Tabet JY, Hittinger L, Thabut G, Jourdain P, Maison P, Tartiere JM, Solal AC OBJECTIVE: We studied prevalence, causes and consequences of worsening Renal function (WRF) during hospitalization for Acute heart Failure (AHF). METHODS: Patients admitted for AHF were enrolled. Patients with severe chronic Renal Failure, cardiogenic shock and contrast medium-induced nephropathy were excluded. WRF was defined as an increase of 25 mumol/l or more in serum creatinine relative to the admission level. Survivors were monitored for 6 months, focusing on deaths and first unscheduled readmissions for heart Failure. RESULTS: Among the included 416 patients, WRF occurred in 152 cases (37%), 5+/-3 days after admission, and two-thirds of patients recovered their baseline Renal function before discharge. Old age, diabetes, hypertension and Acute coronary syndromes increased the risk of WRF. In-hospital furosemide doses as well as discharge treatment were similar in WRF and no-WRF patients. Serum creatinine elevation was the strongest independent determinant of a longer hospital stay (r=0.37, p=0.001). We assessed the effect of aprotinin on the risk of Acute Renal Failure in 423 patients who underwent on-pump cardiac surgery between January 1, 2005 and December 31, 2006. Of these 423 patients, 318 (75.2%) received aprotinin (median dose=3.0 million KIU, standard deviation=2.8 million KIU; interquartile range: 2 million KIU to 4 million KIU). Aprotinin was more likely to be used in patients who did not cease aspirin before surgery, in urgent or emergency surgery, who had impaired left ventricular function, a longer period of bypass and aortic cross-clamp time, and with both coronary artery bypass graft and valvular surgery performed. The overall incidence of Acute Renal Failure requiring dialysis was 2.8%. The use of aprotinin was not associated with a reduction in transfusion nor an increased risk of Renal Failure requiring dialysis, atrial fibrillation, cerebrovascular accident or mortality in the univarate analyses. finally i found my strong subject! i love what im learning about Acute and chronic Renal Failure. maybe its the instructor maybe its the information. my question is this... i understand the differences in symptoms in the phases and causes of Acute Renal Failure. however chronic Renal Failure seems just to be the symptoms of oliguric phase of intrarenal ARF. and then the changes in symptoms with dialysis is this correct? maybe i get it, maybe im trying to make it harder than it really is. i am just looking for some for sure tell tale signs and symptoms that will be determinate of Acute or chronic. thanks for your help. ...

Results: Between 1995 and 2006, 13 patients with co-existing end-stage heart and Renal Failure underwent simultaneous heart and kidney transplantation at the authors' hospital. Heart Failure was secondary to dilated cardiomyopathy in five patients, ischemic cardiomyopathy in three, cardiac allograft vasculopathy in two, and congenital heart disease, cardiac allograft Failure, and Acute myocarditis each in one. Renal Failure was secondary to glomerulonephritis in six patients, heart Failure in two, cyclosporine nephropathy in three, hypertension in one, and systemic lupus erythematosus in one. Eight patients were in UNOS status IA and five patients in UNOS status II before transplantation. The 30-day mortality rate and in-hospital mortality rate were 15% and 38%. Of eight patients in UNOS status IA, seven patients have lived beyond 30 days and three (38%) beyond 1 year. Radiocontrast-Induced Acute Kidney Injury. Nephron Physiol. 2008;109(4):p61-p72 Authors: McCullough PA Many radiographic studies and procedures use iodinated contrast media and consequently pose the risk of contrast-induced Acute kidney injury (AKI). This is an important complication, which accounts for a significant number of cases of hospital-acquired Renal Failure associated increased hospital length of stay and increased mortality. Sustained reductions in Renal blood flow, hypoxic injury, direct cellular toxicity by the contrast media, and superimposed organ injury are all believed to play a role in this form of AKI. Avoidance of dehydration and multimodality prevention measures may reduce rates of this problem in patients at risk. Contrast-induced AKI is likely to remain a significant challenge for specialists in the future since the patient population is aging, chronic kidney disease and diabetes are coming more common, and use of iodinated contrast is growing.
